The Healthy Communities Planning Toolkit is a curated collection of resources for both community planners and public health professionals to advance health outcomes throughout the Miami Valley. This is part of PLAN4Health – Miami Valley’s long-term goal of seeking solutions and building capacity.

Healthy Communities Data Atlas
The Healthy Communities Data Atlas is a collection of mapping applications that explore health in the Miami Valley by examining health through different lenses, all via highly interactive map-based applications. Explore each of the applications by clicking their respective image below.
Composite Health Score
The Composite Health Score is a score assigned to areas in the Miami Valley based on five major community factors. It provides a comprehensive view of health in the Miami Valley. So far, two resources are available for the Composite Health Score; the Composite Health Score Map Gallery and the Composite Health Score Equity Analysis.
